Patient Resources

At our office, we believe it is very important for our patients to feel informed, appreciated, and cared for at all times while visiting our practice. We look forward to offering you quality personalized dentistry, and we invite you to contact us soon to schedule a visit.

Following is more information about our practice protocols.

Financial Options

For your convenience, Dental Care accepts cash, check, Visa®, MasterCard®, Discover®, American Express®, and CareCredit®, and many insurance plans. If you have questions, and for other options and resources, please call and speak to our friendly staff.

We deliver excellent patient care at the most reasonable cost possible to our patients; we ask that patients reserve treatment appointments by paying for their dental services (or estimated co-payment if insurance is involved) at time of scheduling. We will do our best to give you an accurate estimate of your insurance coverage for planned treatment. Any remaining balance due for services rendered will be your responsibility.

Dental Insurance

We accept all PPO dental insurance plans and will file the claims at the time of each appointment.

Savings Plan

We are pleased to offer our patients our own in-office dental savings plan and invite you to call us to learn more about it.

Here at Dental Care, we understand patients’ financial concerns and we are very happy to answer any questions you might regarding your account, and/or our payment options and protocols. Please don’t hesitate to reach out to us by phone at 123-456-7890 should you need assistance or clarification. We are always happy to help you!

Patient Forms

You may complete your new patient paperwork before your first visit. You are not required to fill out these forms in advance, but doing so can help reduce your waiting time.

Click on the appropriate form from the list below, based on if you have dental insurance. All of the forms are in PDF format and can be viewed and printed easily from your computer prior to your appointment. The forms require the Adobe Reader software to view and/or print.
